Mayfield Falls is a real river hike about 45 minutes from Montego Bay. You walk upstream through a narrow gorge with 21 natural pools and several small waterfalls. Expect to climb over rocks, hold onto ropes, swim in cold river water, and get completely soaked. Guides help you up the slippery sections and point out the ferns and caves. It's moderately physical – not a gentle stroll – but most reasonably fit people manage it fine. The water is clear, the jungle is thick, and the crowds are much smaller than Dunn's River. A typical trip from Montego Bay includes transport, entry, a basic lunch, and a drink; figure on spending around $110–$150 per person depending on group size and whether you add horseback riding.
Best time is December to April when the river is lower and clearer. June to November the water can be higher and browner after rains. Skip the horseback option unless you really want it – most people say the walk itself is the highlight. Wear water shoes with good grip (they rent them cheaply on site) and bring a dry bag for your phone. The included lunch is usually forgettable; if you're hungry, eat well before you go. It's a genuinely good half-day nature trip if you like getting wet and seeing pretty jungle scenery without the theme-park feel of the bigger attractions.
